Cognitive-Achievement Relationships

This document serves two purposes:

1. To help generate hypotheses about the child's cognitive strengths and weaknesses based on the child's achievement strengths and weaknesses.

2. To determine the "Academic-Specific ICC" on the PSW-A Data Summary page in X‑BASS.

CHC-Based Teacher Input Form

The teacher form and accompanying spreadsheet will provide qualitative data to support your assessment. Click on the buttons below to make copies. The links will include caution flags to warn you that they contain scripts. The scripts are necessary for functionality and are completely safe.

Making new forms for each student is currently a very tedious process. (Blame Google, not me.) I'm working on a new script that will make it a one-step process.
