CHC Checklist
Use this form to keep track of your scores during PSW testing. This Word document can be edited to align with your usual battery, and you can create different versions for different referrals.
Cognitive-Achievement Relationships
This document serves two purposes:
1. To help generate hypotheses about the child's cognitive strengths and weaknesses based on the child's achievement strengths and weaknesses.
2. To determine the "Academic-Specific ICC" on the PSW-A Data Summary page in X‑BASS.
CHC-Based Teacher Input Form
The teacher form and accompanying spreadsheet will provide qualitative data to support your assessment. Click on the buttons below to make copies. The links will include caution flags to warn you that they contain scripts. You will also get caution flags the first time you use them, and you will have to give permission. The scripts are necessary for functionality and are completely safe.
The process for making new forms for each student will seem a bit tedious at first. (Blame Google, not me.) The good news is that you'll get the hang of it quickly. I do have a script that makes it a one-step process, but there were IT conflicts, and the setup process is so complicated it's barely worth it. If you just follow the instructions on the form for manually making new copies, it will become second nature after a few uses.
Parent Input Form
This form is for parent information. It is not explicitly tied to learning disabilities, but it does address some of the exclusionary factors. While I prefer to interview parents in person, I sometimes use this form when a full interview isn't possible. But in all cases, it's important to make some kind of personal contact during the assessment.
I developed forms in other languages as I needed them. They can be easily translated to English by uploading a PDF of the responses to AI.
